Insurance
I am contracted with a wide range of In- Network Major Medical Insurance Plans as well as many Employee Assistance Programs. All Insurances are subject to verification & authorization. Please contact your Insurance provider directly to inquire about your specific outpatient mental health benefits. Please note that if your annual deductible has not been met then you will be responsible for meeting all expenses until your deductible is satisfied before your insurance company will start reimbursing for benefits.
I recommend asking these questions to your Insurance provider to help determine your benefits:
- Does my health insurance plan include outpatient mental health benefits?
- Do I have a deductible? If so what is the annual deductible? What portion has been met? What portion needs to be satisfied before outpatient mental health benefits are paid to provider?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
Payment
Accepted forms of payment are Cash, Check, Credit or Debit Cards (Visa, Mastercard, American Express & Health Savings Accounts- HSA)
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
